If you have a winning WEEKLY 50/50 number, please provide the following information to claim your prize:

  1. Copy of the front of your driver’s license

  2. The email address used to purchase your ticket, your mailing address and phone number

  3. Completed IRS Form W-9 if you are a main prize winner over $2,500 (Download at www.IRS.gov)
