How to Claim a Prize

If you have a winning BONANZA EXTRAVAGANZA WEEKLY 50/50 number, please provide the following information to claim your prize:

  1. Copy of the front of your driver’s license

  2. The email address used to purchase your ticket, your mailing address and phone number

  3. Completed IRS Form W-9 if you are a main prize winner over $2,500 (Download at www.IRS.gov)

If you have a winning EVENT number, please provide the following information to claim your prize:

  1. Copy of your ticket

  2. Copy of the front of your driver’s license

  3. Your mailing address and phone number

  4. Completed IRS Form W-9 if you are a main prize winner over $2,500 (Download at www.IRS.gov)

  5. The number and time your winning ticket number was drawn on.
